2020-21 Flu Vaccine Returns

The boxes MUST be:

• UNOPENED, FULL BOX (10 syringes ONLY)
• First come First Serve. Only the first 92 FULL, UNOPENED boxes will be accepted as returns.

  (We purchased total 460 boxes of Flucelvax Quad and Afluria syringes for the 2020-2021 season; therefore we can return up to 20% or 92 unopened full boxes.) 

   

  Instruction:

  1. Post how many unopened boxes you are returning.

  2. Drop them off to CKAPhA office anytime Monday through Friday between 9 AM to 5: 30 PM.

  3. Please call CKAPhA Office at (213) 383-3119 ahead to notify of returns.

  Any vaccines that arrived after 3/1/21 cannot be accepted at all.

   

  2020-21 Flu Vaccine Return 기간은 3월1일까지입니다.

  선착순으로 up to 92 boxes 만 받습니다.

  UNOPENED, FULL BOX (10 syringes ONLY) 만 Return 가능하신점 꼭 유의하시기 바랍니다.

  Nationwide Medical Supply 에서 받은 Lot, Expiration date 가 정확하게 일치 해야지만 return을 받아줍니다.

   

  1. 아래에 댓글로  몆 Box 를 return 하는지 POST 해주시고

  2. 월요일부터 금요일 9 AM to 5:30 PM 사이에 약사회 사무실로 직접 DROP-OFF 해주십시요.

  3. 미리 약사회 사무실로 전화를 해서 방문 날짜와 시간을 알려 주시기 바랍니다.

   

  3월1일 이후로 도착한 Vaccine 은 Return 할 수 없습니다. 이 점  이해 해주시기 바랍니다.
